Before your confirmed evaluation, complete your secure intake so your time with Dr. Coon can focus on understanding the problem and planning your care.
Before we meet
Open your patient portal and follow the intake instructions provided for your appointment.
- Describe your symptoms and previous care. Consider one activity you would most like help with.
- Include your questions, current medications and allergies.
- Upload relevant imaging reports and medical records through the secure portal, rather than ordinary email.
If you cannot access the portal or obtain a relevant record, contact NOVA before your visit so the next step can be clarified.
